Upon purchase please private message via Facebook or Email with the following information;

• Baby's First Name:
• Middle Name/Last Name:
• Date of Birth:
• Weight at Birth (Pounds & Ounces):
• Length at Birth (in CM):
• Time of Birth (in am or pm):
• Place of Birth:

 

• Two colours of your choice:
• A photo or image of your choice (From our sample images, your own image, or make a request):
• Icons you'd like between the date (eg hearts, stars. I can do pretty much anything you can imagine, feel free to make a request.)
   

Please Note: Custom orders may take anywhere from 1 day to 3 weeks to complete (depending on item, and  how many orders I currently have) 
I do also have two children to care for, so please bare with me!
